Coordinate your care
A coordinated care team can help make your health care simpler, safer, and more effective. Here's what to know to stay safe and well this season.
Good health is a team effort — so think of yourself as the team manager. Coordinating your care providers and keeping them looped in is key for getting the best possible care.
Find the right providers
Seeing your PCP regularly is a key part of maintaining your health. Visit ibxmedicare.com/providerfinder or call our Member Help Team to find providers near you. Your provider can help prioritize health goals and decide whether you should see any specialists. (With your plan, no referrals are necessary.) If you have health questions between appointments, you can call our Registered Nurse Health Coaches1 at 1-855-332-3102 (TTY/ TDD: 711) 24 hours a day, seven days a week.
Loop in your team
Make sure all your providers have a complete picture of your health. Share your medical history, treatments, and any medications you’re taking with the entire team. This is important because medications prescribed by one provider could interact with those prescribed by another. This way, they can work together to prevent such risks and discuss other alternatives.
Prep for appointments
Start by writing down any new symptoms or changes in your health since your last visit. Then, review your current medications so each provider knows exactly what you’re taking. Also, bring a list of questions for your provider, such as these: Are there any new treatments or specialists you should consider? Should you watch for side effects with any medications? Should you adjust treatments based on recent health changes?
You can also ask for advice on managing conditions such as diabetes, heart disease, arthritis, or depression. Showing up for your appointments prepared helps each doctor provide effective, personalized care.
Encourage interaction
If you’ve had major tests, surgeries, or medication changes, ask your doctors to share updates with one another. (This may require completing a few forms that give them permission to connect about your care.) You can also request a formal care plan that outlines how each provider is involved in your treatment to ensure that your health care team is all in agreement about managing your condition.
Be a record keeper
Your doctors likely share information through electronic health records. But you should still keep a record of your own medical history, including any meds you’re taking, health conditions, surgeries, and vaccination history. With this information on hand, you can quickly inform any of your providers and pharmacists.
1. You have the right to opt in or out of these services. Simply advise your Health Coach or case manager at any time if you wish to end your relationship with them. You may also call the number on the back of your member ID card and ask that you not be contacted by a Health Coach in the future.
