Caregiving can be a thankless task. Spouses, family members, friends, or loved ones who care for you (or whom you care for) qualify as caregivers.
Performing caregiving tasks — helping with doctor visits, medications, groceries, expenses, or planning for the future — can be confusing and overwhelming. Fortunately, we offer caregiver support services from Carallel.1 These include access to caregiving experts called Care Advocates and digital tools that make staying on top of caregiving responsibilities easier.

Care Advocates are available to listen, do research, and chart a path forward that you can feel confident about. They can help caregivers manage a loved one’s health conditions, navigate family dynamics, find in-home care providers, and more.

Carallel is part of your IBX Medicare Advantage plan benefits. There is no cost to use the service.
Who can use Carallel services?
Any IBX Medicare Advantage member, or the family and friends of an IBX member, can use Carallel services. Just have the member ID number ready when you call 1-877-715-0877 (TTY/TDD: 711), Monday through Friday,
8 a.m. to 7 p.m.

Independence Blue Cross offers PPO, HMO-POS, and HMO Medicare Advantage plans with a Medicare contract. Enrollment in Independence Blue Cross PPO, HMO-POS, and HMO Medicare Advantage plans depends on contract renewal.

Authorization for Disclosure of Health Information, Personal Representative, and Appointment of Representative: Learn more at ibxmedicare.com/caregiver.
Authorization for Disclosure of Health Information form — This form allows the member to give permission to our Member Help Team to discuss plan and personal health information with a caregiver. They cannot make any changes to the member’s account information.
Personal Representative form — This form allows the member to give a caregiver full access to their member account and personal health information. It allows IBX to permit the caregiver to request changes to the member’s account and receive personal health information on behalf of the member. Legal documentation is required to be submitted with the completed form.
Appointment of Representative form — This form is only needed when the caregiver is filing an appeal or grievance on the member’s behalf, or speaking with a third party vendor.
