Health Coaching

Health Coaches are registered nurses who are available to answer your health-related questions and address your concerns.1 Available 24/7, year-round, Health Coaches can help you to:

  • Understand your diagnosis.
  • Educate yourself about your condition.
  • Learn to actively manage your health.
  • Make lifestyle choices that can reduce your health risks.
  • Learn self-care skills and how to follow your treatment plan.
  • Prepare for visits and discussions with your doctor.
  • Weigh the pros and cons of important decisions using evidence-based information they can provide.
  • Address everyday health concerns.

Condition Management

In addition to offering access to Health Coaches, condition management provides you with resources that can help you get the information and support you need to make the health decisions that are right for you.2

Case Management

If you are facing a serious illness or medical condition, then case management may be right for you. Independence case management is a free, confidential program offered to all our members. Our case managers, referred to as Health Coaches, are registered nurses and social workers who pair their occupational expertise with knowledge of Independence benefits to offer you support and guidance in dealing with your complex health concerns.

Most often our Health Coaches will reach out to you by telephone or letter to offer their assistance. This generally happens after you’ve had a hospital or rehabilitation stay. However, if you feel you need help from a Health Coach about a health-related issue, call 1-800-ASK-BLUE and leave a message. A Health Coach will return your call as soon as possible.

Baby BluePrints® Maternity Program

If you’re an expectant mother, you want to ensure you have the healthiest pregnancy and delivery possible. Baby BluePrints is our comprehensive, free maternity program that supports pregnant members from the first trimester through delivery to home and continues with the 24/7 availability and phone support of Registered Nurse Health Coaches.

When you enroll in Baby BluePrints, a Health Coach will assess your pregnancy, screen for depression, and answer your questions about taking care of yourself and your baby at this special time. High-risk pregnancies will be referred to a Registered Nurse Health Coach specially trained in maternity care for additional assessment and follow up.

Comprehensive Care Program

Years ago, all doctors made house calls, and there was good reason for it. A doctor could provide more immediate, personalized and family-oriented care in the home setting than in an office. That is still true today.

House calls, as part of the Comprehensive Care Program, are specially designed for our members who will benefit from regular face-to-face visits with a physician or nurse practitioner in the comfort of their home, and 24-hour access to their care team.

How the Program Can Help You

Your care team can include a physician, nurse practitioner, social worker, pharmacist, dietitian, behavioral health specialist, and team of care coordinators who will work with your primary care provider (PCP) to help you receive the care you need in the comfort of your own home. Together, they will help develop a care plan that aligns with your goals, helps you feel better, and helps improve your quality of life.

What You Can Expect From the Program

  • A plan of care that your care team, your PCP, family, and you will develop to help you feel better
  • Face-to-face medical visits as quickly and frequently as you need
  • Access to your program care team 24/7
  • Help managing your medications
  • Coordination of appointments with other health care providers
  • Education about your chronic conditions
  • Home safety evaluation
  • Help finding community resources available to you
  • Advanced Care Planning based on your wishes for your care in the future

How Do You Know if You’re Eligible?

Independence has contracted with an independent company to provide the program. If you’re eligible, you'll be contacted by the company to enroll in the Comprehensive Care Program. Eligible members typically suffer from multiple chronic conditions and may experience frequent hospitalizations. This program is available at no additional cost to eligible members.
