Our region has a large concentration of health systems generating electronic health records, and HealthShare Exchange (HSX) provides a way for health systems and health insurers to communicate more efficiently with each other. HSX allows medical centers, insurers, and practice offices to share clinical records to make patient care more informed.

Social Barrier Index
Independence has an in-house, conceptual model of a Social Determinants of Health (SDOH). It provides guidance to our Health Coaches to screen members who have identified barriers to health care and allows Health Coaches to address those barriers that may impact a member.
Provider Engagement, Analytics & Reporting (PEAR)
The Provider Engagement, Analytics & Reporting (PEAR) portal offers our participating providers* a single point of entry to access information for patients who are members. It connects providers quickly and securely to the plan information they need to deliver high-quality care. Through the various applications within the PEAR portal, providers can easily access the clinical and financial information specific to their provider organization and plan contracts.
iStrat is a risk stratification engine and data repository that helps Independence identify and engage members in their health care experience. It provides in-depth data and analytics from numerous sources. The data is then used to match members with targeted outreach and support designed to help them achieve better health outcomes. Outreach includes:
- Digital and phone messaging
- Email and direct mail
- Individual support from Registered Nurse Health Coaches along with a multi-specialty clinical care team
