Is my child eligible for CHIP if I have other health insurance?
No. To be eligible for CHIP coverage, a child cannot be covered by any other health insurance (must be totally uninsured).

How do I change my child’s Primary Care Physician (PCP)?
To change your child’s PCP, simply call Member Services at 1-800-464-5437.

Does my child need a referral for vision care?
Your child does not need a referral for vision care. Vision care benefits are administered by Davis Vision. A Davis Vision participating provider can be located by calling 1-888-393-2583.

Can I get portions of this coverage for my child that are not covered under my own health insurance?
This coverage cannot be used in conjunction with any other health insurance plan.

I live in another state. Can I get coverage for my children with the Caring Foundation?
No. All states do not offer this program of CHIP coverage. For information about what’s available in your state, call your local Blue Cross and Blue Shield plan or your State Department of Health.

What do I need to do if I have to take my child to the emergency room?
Emergency services do not require a referral for treatment from the PCP. If you believe your child needs emergency services, call 911 or go immediately to the emergency department of the closest hospital.

What documentation must I submit for income verification?
You must attach proof of gross income (before taxes and deductions) received during the last 60 days. View a guide to Acceptable Income Documentation.

How soon will I be notified after you receive my child’s application?
It normally takes 3–4 weeks for applicants to be notified.

Is oral surgery covered?
Oral surgery is covered for the surgical extraction of bone-impacted wisdom teeth, accidental injury, correction of a non-dental physiological condition, and treatment for tumors or cysts.
