Personal health visits

IBX is working with ComplexCare Solutions to improve health outcomes by offering personal health visits at no cost. A personal health visit can help you stay up to date with your health care between your regular doctor appointments. This visit is not a substitute for your doctor’s appointments.

This in-person or virtual appointment is with a licensed medical professional and will last about 45 minutes to one hour. The medical professional will perform a detailed physical exam, review your results, discuss your health history, and address any questions or concerns you have. ComplexCare Solutions will share your exam results with your regular doctor.

As a bonus for completing your personal health visit, you can claim a $50 retail gift card! Participation in this program is not mandatory, and the visit will not affect your health benefits or premiums.

Go paperless

Get your plan information faster. You will get emails or texts instead of mail when we have news to share. That means less paper and less waiting. See things like:

To update how you receive certain documents, log in to the IBX member site at ibx.com/login, visit Account Settings and select Communication Preferences, then choose Go Paperless for Required Communications.

Extra benefits for members with certain health conditions

IBX offers extra benefits to qualifying members enrolled in certain HMO plans. These benefits may help with meals, food and produce, rides to health-related locations, or safety at home.

Benefits vary by plan, so check your Evidence of Coverage, also called your EOC, to see what is included with your plan.

IBX uses information from your health claims and, when needed, documentation from your provider to see if you qualify.

Special Supplemental Benefits for Chronically Ill (SSBCI)

CMS guidelines require members to have one of the eligible chronic conditions listed below and have a condition that:

Is life-threatening or significantly limits the overall health or function,
Has a high-risk of hospitalization or adverse health outcomes,
And requires intensive care coordination

Meals Program (MANNA)

Some members may qualify for meal delivery after they return home from a hospital or care facility stay.

If you qualify, you can get:

3 meals a day
7 days a week
21 meals each week
Meals delivered once a week for 4 weeks

This benefit may be used up to 2 times each calendar year. It cannot be used more than once in any 30-day period.

Eligibility criteria

You may qualify if you:

Have a diagnosis of diabetes and congestive heart failure within the last 5 years, and
One hospital stay, skilled nursing facility stay, or home health care claim, or two doctor or outpatient visit claims that include a diagnosis for the condition.

Or you have:

Diagnosis during the calendar year of female/male breast, colorectal, endometrial, lung, or prostate cancer, and
One hospital stay or skilled nursing facility stay claim, or two doctor or outpatient visit claims that include a diagnosis for the condition.

Available on plans: Keystone 65 Assured HMO, Keystone 65 Essential HMO-POS, Keystone 65 Focus HMO-POS, Keystone 65 Preferred HMO, Keystone 65 Select HMO

Food and produce delivery (FareRx)

Some members may qualify to receive food and produce delivered to their home.

If you qualify, you can receive this benefit for up to 4 consecutive weeks each year (one time per year).

Eligibility criteria

You may qualify if you have a diagnosis of diabetes and depressive disorders.

Available on plans: Keystone 65 Focus, Keystone 65 Preferred, and Keystone 65 Select plans

Help with certain chronic conditions

Transportation (Roundtrip)

Some members may qualify for rides to health-related locations.

If you qualify, you can get up to 24 one-way trips each year (12 round-trip). Rides may include:

Rideshare, such as Lyft or Uber
Taxi
Medical sedan
Wheelchair van (subject to availability)

The type of ride depends on your needs.

Eligibility criteria

You may qualify if you have diagnosis of diabetes and congestive heart failure.

Each one-way ride is limited to 80 miles.

Available on plans: Keystone 65 Focus Rx HMO-POS, Keystone 65 Preferred Rx HMO, and Keystone 65 Select Rx HMO plans

Qualifying members can book and manage your rides through the Roundtrip Rider app, or call 1-877-661-1864 (TTY/TDD: 711). Representatives are available 24/7.

Medical alert system for help at home

Personal Emergency Response Systems (PERS) (Medical Guardian)

Some members may qualify for a personal emergency response system, also called a medical alert system.

This system can help you live more safely at home. It includes 24/7 monitoring, fall detection, and help in an emergency.

Eligibility criteria

You may qualify if you were diagnosed with one of the following health conditions:

Neurologic disorders (cerebral palsy, multiple sclerosis and transverse myelitis, muscular dystrophy, Parkinson's disease, secondary Parkinsonism)
Stroke/transient ischemic attack
Dementia
Fibromyalgia
Chronic pain and fatigue
Mobility impairments; osteoporosis with or without pathological fracture

Available on plans: Keystone 65 Assured HMO, Keystone 65 Essential HMO-POS, Keystone 65 Focus HMO-POS, Keystone 65 Preferred HMO, and Keystone 65 Select HMO plans
