Keystone 65 Basic Rx HMO

- No referrals or deductibles
- Largest network of doctors and hospitals in our coverage area¹
- $0 copay for routine vision and hearing exams once per year, plus coverage for eyewear and hearing aids
- Dental coverage including a $0 copay for a routine exam and cleaning every 6 months and a dental allowance for dentures, fillings, crowns, and much more
- $300 annual flex benefit to cover cost-sharing for covered dental, vision, and hearing benefits, or pay for services or supplies received from any licensed dental, vision, or hearing professional that accepts the IBX Care Card²
