Council for Affordable Quality Healthcare

All providers interested in participating in our network are asked to complete and return a credentialing application kit. Filling out multiple forms can take hours, especially when a practice contracts with multiple health plans.

The Council for Affordable Quality Healthcare (CAQH) application eliminates the need to fill out multiple, redundant and time-consuming forms.

Medical practices often spend thousands of dollars to buy specialized software, hire consultants or even employ full-time staff to manage the administrative burden of reporting provider data. The CAQH application is entirely free to providers, and helps practices save money on administrative resources.

Keep your information current

Keeping practice information up-to-date isn’t just important for credentialing purposes, it’s important for health plan records and directories too. With CAQH, a physician or healthcare provider only needs to update information that has changed. There’s no need to fill out information forms over and over again.

The CAQH electronic credentialing application is free to providers and available on the CAQH website.
