The Personal Health Profile (PHP) provides employees with a personalized, confidential assessment of their health risks along with recommendations for improving specific health behaviors. The questions on the PHP cover a broad range of topics including smoking, exercise, nutrition, sleep habits, stress, and other gender-specific health topics.
Note: The PHP is designed to be an educational tool only. The information provided on the individual participant’s report is a means of identifying potential health risks and guiding the individual towards appropriate lifestyle changes. The profile is not intended to replace a complete medical or mental health assessment.
THE EXECUTIVE SUMMARY REPORT
The Executive Summary Report provides the basis for targeting the specific needs of the member population to assist the employer in planning an appropriate health promotion program for employees. The report also provides baseline health and health-risk data that can be tracked on an annual or biennial basis, to assist with worksite health promotion program design.
The Population Health and Wellness Department will compile and analyze the aggregate data from your employees’ PHPs and generate a comprehensive Executive Summary Report for Benefit or Human Resources management review. These management reports provide employers consolidated information to:
- Design a focused worksite wellness program
- Target health promotion efforts to specific risk areas
- Develop appropriate programs, reflecting the health needs and interests of the employees
- Establish a baseline for evaluation of future activities
Note: 10% employee participation is required before an Executive Summary Report can be generated for groups over 100.
