Connections^SM Health Management Programs
Connections provides information and support to encourage members to take an active role in their care. This program supports members who have everyday health concerns, in addition to those with more serious chronic conditions such as diabetes, asthma, or heart conditions.

Case Management
Case management provides members with health management support. It is a free and confidential program offered to members who are experiencing complex health issues or challenges.

Good 2 B Me Website
Good 2 B Me helps teens and adolescents — and their parents — learn about the major physical, emotional, and social issues that affect them and gain the confidence to make smart choices for their health today and in the years ahead. A main focus of the website is adolescent and teen immunizations and the crucial role they play in protecting kids’ health.

The Good Health Club^SM
We recently joined forces with the Blue Cross and Blue Shield Association to tackle childhood obesity and diabetes. The Good Health Club has a listing of materials that can help kids get started down a path of healthy living.
