Connections is designed to help members become better informed health care consumers. Members with chronic conditions such as diabetes, lung or breathing problems, or heart conditions can learn how to improve their health and overall quality of life. In addition, this program supports members who are facing a significant medical decision, as well as members who have everyday health concerns.

Making important medical decisions is a responsibility you share with your doctor. This program can help you get the support and encouragement you need to manage your care and make informed health decisions.

Connections features

• access to a Health Coach, 24 hours a day, seven days a week, 365 days a year;
• personalized calls from your Health Coach about your chronic condition and other health concerns;
• educational materials and health reminders mailed to your home.

The Dialog Center^SM

The Dialog Center, made available through Connections, features a messaging center to request information from a Health Coach and gives you access to health tools, decision support, and the Healthwise^® Knowledgebase* — an online encyclopedia with thousands of easy-to-understand articles.

Participation in Connections is voluntary, and the program is available at no cost to Keystone Health Plan East HMO, Point-Of-Service, or Personal Choice^® members.**

Members must log in or register for ibxpress to access the Connections Dialog Center.
