If you are already enrolled in the Standard or Basic Option plans and don’t want or need to make any changes, you do not need to do anything. You will automatically be re-enrolled, and your current coverage will continue in 2010. However, if you are currently enrolled in a Keystone Health Plan East plan, you must select another plan, or you will not have coverage as of January 1, 2010.

If you are enrolling for the first time or need to make changes to your current plan, follow these steps:

For FEHB Medical Plans:

Choose a medical plan.
Choose your desired family status (self or self & family), the plan you’d like to join, and make a note of the corresponding FEHB enrollment code:

Enroll online using your agency’s method^*:

Employee Express — see a list of agencies participating in Employee Express.
Department of Defense — DoD automated enrollment systems.
U. S. Postal Service — PostalEASE system or the telephone enrollment system.
Department of Energy — DOE automated systems.
Health and Human Services and Environmental Protection — MyPay.
Employees of agencies payrolled by the National Finance Center — Employee Personal Page.

Fill out the SF 2809 Form and provide a copy to your Human Resource office.

For FEDVIP Vision and Dental Plans

Go to www.benefeds.com or call 1-877-888-3337 to enroll.
