Grant program overview

Independence Blue Cross is proud to offer a competitive Clinical Care Innovation Grant Program (Grant Program). The Grant Program is intended to foster new, innovative approaches to care delivery that will ultimately help improve the health and well-being of its members.

Independence will award five grants in 2021 with an average of $200,000 per grant to health systems and large specialty groups with value-based care contracts with Independence. The awarding of grants and disbursement of grant funding is contingent upon Institutional Review Board approval. All grant proposals and awards will need to comply with all applicable state and federal requirements including, but not limited to, all requirements and regulations imposed by The Centers for Medicare and Medicaid Services.

Please note: Each entity is permitted up to three submissions for the 2021 cycle. The deadline for submissions is December 31, 2020.

Grant criteria

For the Grant Program, we are seeking submissions that focus on the following:

  • New, innovative approaches to care delivery (i.e., not having been done elsewhere), including those that leverage technology
  • Clinical approaches that result in improved quality, patient experience, and access to care, and lower health care costs
  • Approaches that bring patient care to the home setting if needed
  • Approaches/capabilities that are not currently supported by Independence’s provider incentive programs

If you’re interested in applying for the Grant Program, please keep the following dates in mind:

  • Submission deadline: December 31, 2020
  • Internal review by Independence review committee: January 4 to 11, 2021
  • Finalist notified: January 12, 2021
  • “Pitch Day” for finalists: January 26, 2021
  • Grant winners notified: January 27, 2021
  • Grant Program launch: February/March 2021

What you need to apply

Interested in applying for the Grant Program? Your grant proposal should include the following information:

  1. Pilot scope/problem statement
  2. A description of how your clinical approach is new (i.e., targets improvement)
  3. A description of the use of technology or other advancements to improve care
  4. Evaluation metrics, including clinical, financial, and patient experience
  5. A program description, including duration and how the study will be administered. Be sure to include study personnel, timeline and deliverables, demographic sample profiles, and a description of a COVID-19 contingency plan.
  6. Sustainability approach post-pilot and plan to scale to full target population(s)
  7. A detailed funding request plan and description of how funds will be used
  8. Intended publication/manuscript development plans

  1. Please review the Clinical Care Innovation Grant Program information (above).
  2. Open the Grant Submission Form¹ and complete the required sections. Note: If you have documents that are related to the sections of the Grant Submission Form, you will have the ability to attach them at the time of submission.
  3. Save the Grant Submission Form locally on your device.
  4. Attach the local copy of your Grant Submission Form along with other supporting documents (e.g., study description/approach, IRB approval) or documents that allow you to fully answer the sections of the Grant Submission Form (e.g., exceeded character limits in form) in an email. Your email should be addressed to Please provide clear instructions about linking the applicable sections of the Grant Submission Form to each of the attached documents. Additionally, please include any questions you may have in the body of the email and we will reply in a timely manner.
  5. Once the Grant Submission Form and all related documents are attached to the email, the email may be sent.
