BlueCard^® Coordination of Benefits Questionnaire

Make sure your patients get the care they need covered. The BlueCard Coordination of Benefits questionnaire helps us determine which members have coverage under multiple insurance plans, and which plan is responsible for covering which medical costs. Coordination makes sure each claim is paid quickly and properly. We ask that members complete this questionnaire as soon as possible if they believe they have more than one health insurance plan to avoid billing issues.
