If you have changed your payment name, address, or tax ID number during the past year, please submit an updated Form W-9 to Independence Blue Cross before December 31, 2009. This will ensure that you receive your 1099 form when they are distributed in January 2010.
Please complete the Form W-9 and fax it to 215-761-0337 by December 31, 2009.
