PPO Program ID Cards and Claims Submissions

Beginning January 1, 2010, your office may see patients that have “PPO Program” on their ID cards rather than “Personal Choice®”. These ID cards are issued to members enrolled in Personal Choice through certain national/self-funded accounts.

Follow the current Personal Choice claims submission processes for PPO Program ID card Members. You may check member eligibility and claims status through the NaviNet® web portal.
