Clinical Insights Now Available

The 2009-10 Clinical Insights: Effective Care for Patients with Chronic Conditions is now available. Clinical Insights highlight topics that Health Coaches from the ConnectionsSM Health Management Program may discuss with members who have the following conditions:

  • asthma;
  • permanent (chronic) atrial fibrillation (newly added);
  • chronic obstructive pulmonary disease;
  • coronary heart disease;
  • diabetes;
  • heart failure;
  • primary and secondary stroke prevention (newly added).
