The 2009-10 Clinical Insights: Effective Care for Patients with Chronic Conditions is now available. Clinical Insights highlight topics that Health Coaches from the Connections^SM Health Management Program may discuss with members who have the following conditions:

• asthma;
• permanent (chronic) atrial fibrillation (newly added);
• chronic obstructive pulmonary disease;
• coronary heart disease;
• diabetes;
• heart failure;
• primary and secondary stroke prevention (newly added).
