The last day to apply for a 2014 individual or family health plan is March 31. We are here to help you review your options and choose a plan that best fits your needs and budget. Our licensed representatives are available to answer your questions and help you apply for coverage before the deadline. Use this helpful checklist to make sure you have all the information you need and are ready to apply.

Application Checklist

Here’s a quick checklist of what to bring with you if you want to apply for a plan. The application process takes between 30-60 minutes. To help you complete the application, make sure you are prepared with the following for all members of the family you wish to enroll:

  • Social Security Numbers (or documented numbers for any legal immigrants who need insurance);
  • address information;
  • birth dates;
  • information on tobacco use;
  • income information (paystubs or W-2 Forms, Wage and Tax Statements);
  • employer information including employer name, address, and phone numbers;
  • policy numbers for any health insurance plan you currently have;
  • information from your employer about any job-related health insurance available.
