It is Your Responsibility to:
- Know about your benefits and the proper procedures to follow to obtain those benefits. Look through this benefits handbook. Keep it where you can refer to it.
- Call the Caring Foundation at 1-800-464-5437 if you have trouble understanding anything in this handbook.
- Make premium payments on time.
- Identify yourself as a Keystone member before obtaining covered medical services.
- Call the Caring Foundation at 1-800-464-5437 and tell us when there are any changes in your status. For example, please notify us if:
- your income changes
- your address changes
- your family size changes
- you begin to receive other health insurance benefits
- you become eligible for Medicare or Medicaid
- there are errors on your ID Card
- you lose your ID Card
- Communicate, to the extent possible, information participating providers need in order to care for you.
- Follow the plans and instructions for your care that you have agreed on with your practitioner.
- Ask questions to assure understanding of the explanations and instructions given.
- Keep scheduled appointments or give adequate notice of delay or cancellation.
- Treat others with the same respect and courtesy expected for oneself.

Subrogation
In the event that legal grounds for the recovery of health care costs exist (such as when an illness or injury is caused by the negligence or wrong-doing of another party), Keystone has the right to seek recovery of such costs, unless prohibited by statute or regulation. When requested, the member must cooperate with Keystone to provide information, sign necessary documents and take any action necessary to protect and assure the subrogation rights of Keystone.
