3. Select the network coinsurance level for facility/ancillary care

The coinsurance applies to hospital services, outpatient surgery, and emergency room care. Select from two levels of coinsurance and out-of-pocket maximums. View network coinsurance options.

Keystone Direct POS and Personal Choice PPO plans allow members to obtain care out-of-network. All Flex Deductible plans use the same cost-sharing schedule for out-of-network benefits. View cost-sharing schedule.

4. Select prescription drug, dental, vision, and life benefits
