Making a difference — Independence Blue Cross’ integrated approach to member wellness

We engage members through a variety of targeted outreach programs designed to help them change unhealthy behaviors and understand their health concerns. The type and frequency of the outreach depends on the member’s overall health and specific needs. For example, we proactively identify and reach out to members with chronic illnesses, such as diabetes, that benefit from closely monitored intervention programs, such as ConnectionsSM. We also identify members who may be at risk for significant health issues but are not candidates for chronic disease or care management programs and may need only periodic reminders.

Our integrated approach:

  • identify potential health risks early on and prevent complications;
  • provide members with the right care, at the right time, at the right place;
  • identify gaps in members’ care and get them back on track;
  • provide members with incentives to change unhealthy behaviors.

Workplace Solutions — Integrating Individual Support with Workplace Wellness

We work with employers to integrate the individualized support we provide to employees with workplace solutions for improving health. Resources include:

  • Healthy LifestylesSM Promotion Guide. A Web-based program that helps employers provide effective health education and wellness information to employees. There are 12 pre-planned campaigns, or employers can create their own campaign using the posters, articles, and turnkey programs.
  • Wellness Partners®. We team up with employers to bring targeted wellness programs to the workplace. Some of the features of the Wellness Partners program include employer toolkits, preventive health screenings, awareness seminars, and interactive kiosk displays.
