Supplemental dental coverage for groups

We offer access to a range of United Concordia dental products. You can choose from one of the following options:*

Concordia Plus:

• a dental managed care plan;
• available to groups of two or more;
• statewide network of more than 385 primary dentists and nearly 400 specialists;¹
• no participation requirements;
• eight standard options available.

Concordia Preferred:

• a dental preferred provider organization;
• available to community and experience-rated groups;
• network of nearly 2,500 dentists and specialists;¹
• in-network and out-of-network coinsurance levels apply;
• quality dental care emphasized through advisor reviews, practice pattern monitoring, and strict selection criteria;
• 75 percent of your group's eligible members must participate.

Concordia Flex:

• traditional fee-for-service plan;
• available to community and experience-rated groups;
• statewide network of more than 4,800 dentists and specialists;¹
• cost-savings at an average of 30 percent;²
• four standard options available to groups of 2-50 members;
• 75 percent of your group's eligible members must participate.
