Jefferson School of Population Health sponsors seminars on health and wellness and medical homes

Philadelphia, PA - February 27, 2012 - Patient-centered medical homes — one of the most exciting concepts in health care aimed at improving quality — is one of the many subjects being addressed at a national meeting being held today in Philadelphia. Independence Blue Cross (IBC) chief medical officer Richard L. Snyder, M.D. and other local leaders in the health care field will participate in a panel to discuss how medical homes help patients better manage their chronic conditions, improve care coordination through a team approach, and enhance patient and physician satisfaction.
