If you have a new producer who is interested in selling Independence Blue Cross’s individual medically underwritten products, please have him or her complete the producer appointment form in the link below.

Please note: Prospective agents must submit the following documents prior to solicitation/sale of business in order to be appointed as an agent of IBC. Agents/Agencies must be appointed before the effective date of any business submission. All producer licensing documentation must be submitted by each primary agency through which the producer will submit business. Licensing documentation cannot be processed if it is submitted by the producer directly.

Requirements

Individual Agents

  • Complete the Change/New Request form for primary agents/agencies.
  • Attach a copy of the PA Certificate of Qualification (license)

Agencies

  • Complete the Change/New Request form for primary agents/agencies
  • Include copies of PA Certification for:
    • agency
    • designated licensee (qualifying acting officer)
    • soliciting/selling agents of the agency

Agents/Agencies must notify the IBC Licensing Department immediately of any change in the status of their license, i.e., address, termination, suspension, residency status, or expiration. This notification should be sent to the address noted below.

Completed Forms

Completed Producer Appointment forms should be sent to IBC via regular mail, email, or fax.
